Referring a Case

Putting LMA's expertise to work for you is simple.

• First, you may phone in a referral at 877-511-6642.
• Alternatively, if you register you may download a form to fill out and email back to us at gmosher@lmasettlements.com.
• At a minimum, we will need to know the following information about the claimant:
  • Name and date of birth
  • State of residence
  • Bodily injury or Workers Compensation case
  • Type and severity of injury
  • Date of loss
  • Dependents
• Based on the above data, LMA will prepare structured settlement annuity proposals, or "quotes."
• At your instruction, LMA will initiate a discussion of settlement strategies tailored to the claimant's future needs.
